Nel saw what Ulquiorra was doing and winced, closing her eyes and looking away, but that didn't stop her hearing the noise. It didn't seem to hurt, but that didn't stop it from looking and sounding horribly visceral and... squelchy.
She wasn't normally squeamish but someone popping the eyeball right out of his own head was more than she could take.
